A Personal Health Record (PHR) Address is a self-designated username that is required to log in to a Health Information Exchange and Consent Manager (HIE-CM). Each ABHA number must be linked to a consent manager in order to enable data sharing. At this time, all ABHA number users are able to create their own ABHA address during the ABHA number sign-up process.
To create an ABHA number, you can register using your Aadhaar number. Simply provide your name, year of birth, gender, address, and mobile number. If using Aadhaar, ensure your mobile number is linked to it for OTP verification.
No, the ABHA number creation process involves voluntary usage of the Aadhaar. If you do not wish to use your Aadhaar number, you can simply use your mobile number and provide the necessary self-declared information.

ABDM values user privacy and thinks that people have the right to own their data and make informed decisions about how it is used. They recognise that healthcare is a sensitive subject and do not wish to prevent users from associating different sets of health data to different ABHA addresses if they so desire.

They do, however, advise consumers to create and utilise only one ABHA number in order to develop a more comprehensive and continuous healthcare plan. Users can make more educated healthcare decisions with a consistent, long-term health history linked to a single ABHA number.

The registration process is quick and easy, taking less than 10 minutes to complete. All you need to do is provide your basic information and authenticate your mobile number or Aadhaar number.
Currently, you do not need to submit any physical documents in order to get an ABHA number. The process is completely paperless and hassle-free.
Yes. The Health ID, now known as the Ayushman Bharat Health Account (ABHA), is a unique identification number consisting of 14 digits that can be linked to your Aadhaar number. With ABHA, you can digitally store, save, and share your medical records with healthcare providers and insurance companies.

ABDM now offers a feature that allows users to opt-out of the ABDM system. With this feature, you can either permanently delete your ABHA number or temporarily deactivate it. If you choose to deactivate your ABHA number, you can reactivate it at a later time. However, if you opt for permanent deletion, your ABHA number and all associated demographic details will be erased permanently and cannot be retrieved.

You will no longer be able to access ABDM applications or any health records through the ABDM network using a deleted ABHA number. If you deactivate your ABHA number, you will lose access to all ABDM applications for the duration of the deactivation period. Until you reactivate your ABHA number, you will not be able to share it with any healthcare facility or access your health records through the ABDM network.

You can reach the ABHA Digital Health Card Department by calling their toll-free number: 1800114477.

ABDM does not keep any of your health records in its own system. Instead, these records are kept by healthcare information providers such as Health-e according to their own retention policies. ABDM only accesses and shares your records over its network with your explicit permission, using encryption to protect your privacy.

Health-e has a strict policy to only share or display your data with third parties that are concerned in the ecosystem. Linking of ABHA ID on Health-e is not mandatory, however it is encouraged so that you can have a medical history that is unique to you and align all your medical data on ONE platform.
